Please read the instructions below before proceeding with any New Hire Documents that are linked below.
- Please only complete the documents that you were instructed to complete.
- Each link will open a fill-able document in a new browser window.
- Your signature on these documents will be requested during orientation.
- You can choose to complete the document(s) in your web browser, or print a copy to fill out by hand.
- RIGHT CLICK ON THE DOCUMENT LINK THEN CHOOSE “SAVE AS” OR “PRINT”
- ‘Save As’ will place a copy on your computer that you can fill out using a keyboard
- Use ‘Print’ if you would prefer to fill out the application by hand
- SAVE THE DOCUMENTS TO YOUR COMPUTER PRIOR TO FILLING IT IN OR YOU WILL LOSE YOUR ENTRIES!!
- If you saved the document(s), email to email@example.com, we will print them out for you, to be signed at the orientation.
- If you printed the document(s), fax to 970-563-4504, or drop off a copy to the SUCAP office at 285 Lakin St, in Ignacio, Co.
